This ACMG Membership Reinstatement Application Form will guide you through the steps needed to reinstate your ACMG designation(s).

If you have been inactive in or resigned from a designation for one year or less, you will only need to confirm whether you have fulfilled the minimum number of PP and CPD hours for each certification you hold. Member Services will then reinstate your designation(s) so that you can complete the membership renewal process.

If you have been inactive or resigned from a designation for more than one year, or you indicate that you do not currently have the PP and CPD hours you will be required to upload a PDF copy of your resume, indicating your total number of PP hours, including details about any employment, your employer's contact information, and your total number of CPD hours and evidence of registration for any training courses you have recently attended or plan on attending. The Technical Committee will review your application, and the Technical Director will respond to your application as soon as possible.
