First Aid Certification Form

Please upload a copy of your valid first aid certificate with the first aid course provider, level of first aid and expiry date clearly visible (include both sides of your first aid certificate on the uploaded file if necessary). If your first aid certificate is no longer valid, please upload proof of enrolment in an appropriate recertification course beginning within 90 days of the lapsed certificate's expiry date. ACMG staff will verify the details on your update member profile.
